We understand that you may be putting the “finishing touches” on exhibits leading up to the fair. Please register all items you expect to have evaluated. Stop at the Information Table to delete exhibits you did not bring for evaluation.

This is the parent section of the health form. Health forms are required to be completed and submitted for program participation. Incomplete health form information may result in the inability for your child to participate. Both over the counter medications and prescriptions must be signed off on by your healthcare professional.

Primitive Pursuits-Scholarship Application
Scholarships are generated with our choose-your-price sliding scale fee format, collected and distributed on a program-to-program basis. Scholarships are awarded on a rolling basis, pending available funds. We review applications every 2-3 weeks, and notify applicants soon thereafter. Primitive Pursuits -Summer Health Form
This is the parent section of the health form. Health forms are considered completed once this form and the Medication/ Immunization records are received completed. The immunization records must be uploaded to your Account. Incomplete health form information may result in the inability for your camper to participate in camp. Both over the counter medications and prescriptions must be signed off on by your healthcare professional.

Groton Summer Camp Health Form
This is the parent section of the health form. Health forms are considered completed once this form and the Medication/ Immunization form are received completed. Incomplete health form information may result in the inability for your camper to participate in camp. Both over the counter medications and prescriptions must be signed off on by your healthcare professional. A PRN order must be obtained by the Camper’s physician, Camper must be able to self-administer the medication, which must come in the original container with only the specific dosages necessary for the duration of the program. Parent/Guardian must sign the PRN.
