OVERNIGHT CAMP REQUIRED FORM: Camp Medical Form 2025
The medical form is a self-assessment of medical history. This form must be completed by the adult participant or by the parent/guardian of a minor. It remains an incomplete task until it is audited by staff. You only need to submit one time per winter camp season unless there is a change in medical history.

This form is to provide your travel arrangements to and from camp December 13-15, 2024. Do not fill out this form unless you have registered or are in the process of registering for the December callback camp.
