Camper Health HistoryAfter completing the form online, to print the health history form to give to your physician, do the following:
(1) go to "your account" in Ultracamp
(2) click your child's name
(3) go to "my forms"
(4) click the printer icon beside the health history form
(5) smile after a job well done!
