We may need some more information to better support your camper(s) this summer. Burn Brae Day Camp has a unique program, where our campers travel more independently between their activities. Please fill out the following form for each of camper individually.

Camper/Apprentice Health Form

Majors
Please select a major (AM class) *Please note that theater majors must be present for at least 2 consecutive weeks and present for week 4 and/or 8.

Medication Administration
As far as over-the-counter medications, we will only administer Tylenol for headaches, Ibuprofen for muscle aches/cramps, and Benadryl for allergy symptoms. In the RARE event of an emergency, care will be provided onsite, and the camper will be transported to Abington Hospital ER via ambulance if it is determined that further medical attention is necessary. You will be notified IMMEDIATELY if this situation arises.
