Code of Conduct Form (All Day & Jr Camp)
Our priority at 1870 Farm Camp is to facilitate a fun and magical experience while keeping our campers and counselors safe. Both counselors and campers have a responsibility to conduct themselves in a manner that provides a physically and emotionally safe atmosphere for all our participants. It is our responsibility as a staff to support your child by being role models, following safety precautions, and managing behavioral incidents in a timely and appropriate manner.

Optional 1870 Farm Group Request Form (All Day Camp)
We will not be accepting any more responses to this form after the Wednesday (at 5:00PM) before your registered week of camp. For example, if you are scheduled for the second week of camp starting on 6/19, we will not be taking any more group requests after Wednesday (6/14) at 5:00PM.

JVA Health Form (Summer of 2024)
This health questionnaire is required for all Junior Vet Academy campers. Junior Vet Academy follows requirements set forth by North Carolina Public Schools regarding required vaccinations. It must be completed prior to the start of your session. If you would prefer a paper copy to print and mail a PDF is available on our website which is linked below.
