Staff Reference Form 2
As part of your application, you also need to get 3 people to complete LPC's Staff/CLT Reference Form in order for us to start processing your application. References can not be from minors or relatives and at least one must be from your current home church pastor or the executive director at an organization where you previously served in children's ministry and to whom you have been well known for a period of at least 1 year.
