Designation of Legal Guardian Form
This form is ONLY required if you live further than a day's drive away from Birch Creek's campus in Egg Harbor, WI. Please email this form to an adult who will be responsible for and able to pick up your child in the event of an emergency within a day as described in the Communicable Disease Evacuation Policy.

Form required for each student once accepted.

Scholarship Application
Scholarship form information is collected to make a determination on the amount of scholarship dollars assigned to a student. Adjusted gross income for 2024 (AGI), number of dependents and an audition URL link to a YouTube video/recording are required. Please wait to complete this until your 2024 taxes are filed.

Shuttle Service
This form is required for anyone needing to be picked up from the Green Bay airport (Austin Straubel/GRB) or the Green Bay bus station (Greyhound Station). Due: Not less than 30 days prior to start of session
