2025 Assumption of Risk, Waiver and Release from Liability
The electronically signed parent/guardian understands that his/her daughter’s participation in the Pilot Summer Academy 2024 is voluntary. As such, the parent/guardian assumes all risks which are foreseeable and involved with or may arise out of his/her daughter's voluntary participation.

2025 Photo Release
I/We grant the Academy of Our Lady of Peace and/or its agents, permission to videotape, photograph, record, livestream my daughter’s likeness and/or voice at the Academy of Our Lady of Peace for the purpose of obtaining live or still images or voice commentary for school publications and/or media productions. The purpose of said efforts would be to demonstrate the qualities of the Academy of Our Lady of Peace its academic, religious, and learning environment or convey information about the school. Productions, ads, publications, etc., would be shown to students, prospective students, parents, supporters, and the larger San Diego community, and is not intended for commercial resale.

T-Shirt Size
In order to help us in ordering Pilot Summer Academy t-shirts for all campers, please indicate the best size for your daughter's camp t-shirt. If you have a special request, or additional information we should know to help your daughter feel her best, please indicate that.

OLP Marketplace Purchases
Permission for campers to purchase food items at one of the two OLP marketplace locations. Campers will need to bring a credit card or a method of using Apple Pay. These locations do not take cash.

Friend Request
Request a specific friend to be placed in a group for your Pilot Summer Academy experience; we will do our best to accommodate the request; your friend should be in the same grade, we will not be able to accommodate a request for a friend with greater than 1 grade separation
