This is a list of the hikes we do at Hawkeye. Please check off which hikes you've done while at camp.

International Insurance Release
This form is provided to give an overview of insurance coverage and realities at camp, while a part of camp transportation (i.e. with a camp chaperon), and on out-of-camp Wilderness Adventure Trips. Hawkeye provides coverage for our international campers and all staff for some acute situations resulting from accidents. However there are and have been situations that occur at camp that do not fall under the purview of our insurance coverage. For this reason we strongly recommend that international campers purchase travel insurance prior to the commencement of their time with camp. Travel insurance does provide certain coverage relating to luggage and travel that camp does not in addition to medical instances.

Liability Wavier
THIS IS A LEGALLY BINDING RELEASE, WAIVER AND INDEMNIFICATION OF LIABILITY, AND EXPRESSES ASSUMPTION OF RISK.

Photo Release Agreement
Signing this form gives Camp Hawkeye permission to take pictures of your child and use them for any type of promotional marketing, post on Facebook and use for print materials.

Pre-Camp Letter
This form is to be completed by the Camper/CiT. It will allow the counselors to identify commonalities between the cabin mates and make the their time at camp even more excellent!

This is to keep track of the program area beads each camper and CiT receive while at Hawkeye.
