Please complete this form no later than the end of the day on Wednesday before your camp starts! Note: we share these forms with our Seasonal Naturalist Staff, all trained and certified in CPR and First Aid at the least. Some may be WFR Certified. We share this form with our trained staff so that they can best assist your camper on a day to day basis as well as in case of emergency.
