Summer Staff Reference Form 1 (Church Leader)
Reference form for those applying to serving at Living Waters Bible Camp as summer camp. This reference should be completed by an Elder, Teacher, Pastor, or Youth Leader at your home church.

Summer Staff Reference Form 2 (Mature Christian)
Reference form for those applying to serving at Living Waters Bible Camp as summer camp. This reference should be completed by an older Christian that applicant has known for some time.

Summer Staff Reference Form 3 (Personal)
Reference form for those applying to serving at Living Waters Bible Camp as summer camp. This reference should be completed by a personal reference.

Living Waters Bible Camp- Camper Care Form
The Camper Care Form helps camp staff to serve your camper(s) specifically while at camp.

Living Waters Bible Camp- Health Form
Health Forms are required for all youth camp sessions.

Living Waters Bible Camp- Risk and Release Form
A basic risk and release waiver that is required for all volunteers and guests to complete.

Missions Trip Medical Authorization
Allows LWBC staff to consent to any medical needs of a participant during the Missions Trip.

TST (Teen Service Team) Application
Teen Service Team (TST) is a program that gives the opportunity for 13-15-year-old teens to serve at camp for one or two two-week sessions.

Foundation Camp Application
This application form is for those desiring to attend Foundation Camp. An application must be received and camper accepted before registration can be processed.

This application form is for those desiring to attend Foundation Camp a second time. An application must be received and camper accepted before registration can be processed.

Teleios Application
Teleios is an intensive 10 month, in-residence training program designed to cultivate in each student the character of Christ, so that the power of His life is released to live and work through them. The training consists of a mix of intensive Bible studies, ministry skills training, personal development, and service, all under the watchful care of seasoned mentors. In order to enter the program an application must be completed and accepted.
