All residential students must read and sign a Heartland Housing agreement each semester. Please know that a parent or guardian must sign the agreement as well.

All Campus Vehicle Information Form
All SEU Ohio faculty, staff and students must have a current vehicle form on file to have a vehicle at Heartland. Please ensure that this information is complete in order to obtain a parking pass.

All Student Emergency Medical Form
All students participating in Student Life Activities both on and off campus must have a current emergency medical form and insurance card on file. Please complete form and upload a current copy of your medical insurance card.

Dormitory Key and Facility Key Card Access Agreement
All students must complete a Heartland dormitory key and facility key card access agreement prior to obtaining their student ID.

Intramural Release and Waiver of Liability and Hold Harmless Agreement
All students wishing to participate in Welcome Week activities, Fire athletics and other student life events, OMN events including but not limited to REACH, PK retreat, etc. must have a current completed waiver on file.

Heartland Conference Retreat Center Activities & Program Participation Agreement
All students participating in activities at Heartland must have a signed activities & program participation agreement on file. Please note that if you are under the age of 18, you must download a copy of the agreement and upload a completed copy with parent/guardian signatures as well.
