Camper Flight Information
FLIGHT INFORMATION IS REQUIRED BY MAY 15. This form is only for campers who need transportation to and/or from the airport. Please direct any questions to olivia@camptwitchandshout.org. NOTE: ARRIVALS ON SUNDAY, JUNE 29ND MUST BE BETWEEN 10AM AND 2PM (EST). DEPARTURES MUST BE BETWEEN 2PM AND 6PM (EST) ON FRIDAY JULY, 4TH. Charter bus fees will be around $50 each way, and will applied after confirming your camper's flight details.

Medical Team Pre-Camp Contact Request Form
Please complete the following form to request direct contact via phone call from the CTAS Medical Team prior to CTAS 2025. This call will be to discuss your camper's needs, medications, health concerns, and care planning. Contact will occur at least within one week to the beginning of camp. We ask that you continue to update your camper's medical information on your camper's medical google sheet as changes occur. We will be using the CAMPER MEDICAL/MEDICATIONS FORM as the ONLY reference for your child's medications. If your camper has any changes to their medications, diagnoses, or medical status since completing your child's application PLEASE CONTACT OLIVIA (olivia@camptwitchandshout.org, 919-931-5304) TO MAKE SURE THEIR FILE IS UPDATED.
