Camper Health Form
ALL campers except for Diabetes campers should complete the Camper Health Form.
Media Consent Form
ALL campers should complete the Media Consent Form.
T-Shirt Form
ALL campers should complete the T-shirt form.

All overnight campers should complete the Information for Counselors Form.
Diabetes Information Intake Form
This is our intake form for our Diabetes Campers. Please note that a more extensive health form will become available on April 15th at 9am. This form HAS to be filled out two weeks prior to camp.
