| Q: | Where can I find Health Form H2? |
| A: | Health Form H2 can be found in the document center. To find the document center please click on the three bars in the top left-hand corner. In the pop-up menu select document center. Click on the Health Form H2 link. Please click on download document link. This will show the form in your browser so that you can save to your computer or for you to be able to print out.

| Q: | How do I upload Health Form H2, immunizations, and physical documents? |
| A: | To upload your Health Form H2, immunizations, and physical please click on the three bars in the top left-hand corner of the screen. In the pop-up menu please select document center. Please click on the upload document button. Click on the Health Form H2 link. Select the camper the document is for under the select individual category. Below the select individual category please click on the browse button. This will open a new window that will allow you to locate your saved form(s) to upload. Once you have selected the files select open in the right lower corner of the window. The window will close to allow you to click on the Upload Document button. Once you have uploaded your form(s) they will be visible with in the document center.

| Q: | Can I take a photo of my forms? |
| A: | Please do not take a photograph of your forms to upload to UltraCamp. Please scan all forms into your computer or phone as photographs are often not clear and are hard to read. If we can not read the forms this can impact the safety of your camper.

| Q: | Can I email you my camper's forms? |
| A: | Please do not email your camper forms as it is not as secure as uploading them to UltraCamp. You may however send them by mail to our main office to the attention of Dayna Karius Camp Registrar.
