UltraCamp functions best on a computer and is not always compatible on tablets or mobile devices. If you do not have access to a computer, or need support in completing the application, please reach out to camp@mdausa.org. Language translation services are also available.

We appreciate your willingness to help provide a safe and fun-filled week of MDA Summer Camp for children with neuromuscular disease. Please be sure the information provided in this questionnaire is correct and complete. Any false statement or misrepresentation of the facts called for on this questionnaire or any unsatisfactory reference check will be cause for denial of your questionnaire or your immediate dismissal from camp at MDA’s sole discretion.
