Athletic Physical
Description
Athletic physicals will be held from 8:30 am - 1:00 pm on Wednesday, June 4th.

This Sports physical will meet IHSA requirements for the summer workouts as well as sports for the 2025-2026 season. Classes of 2026, 2027, and 2028 are welcome. Please select one of the appointment times listed.

Please note: A signed and completed Health History Physical form MUST be brought to the Sports Physical Day.

(You will need the login information you have created when signing up for AP testing, bus tickets, summer school/camps. If you haven't signed up, you will be able to create an account at checkout.)
