We appreciate your interest in MDA Family Getaways! It is important to read all the information below, and throughout this application, thoroughly.

At least one person in the family unit must be a registered member with MDA. An MDA Community Member is anyone who has been diagnosed with a form of muscular dystrophy, ALS or a related neuromuscular disease and who resides in the United States or Puerto Rico. By completing this application, you and the members of your family unit will be automatically registered as a member with MDA and agree to receiving email and/or phone communication from MDA. There is no fee associated with your MDA membership. For more information, please visit mda.org/join-mda.

A ‘family unit’ includes an MDA Community Member and their immediate family. Participants can contact MDA to discuss any other individuals they would like to be considered to attend as part of their family unit. The number of individuals within a family unit will vary by session, based on facility accommodations. All minors must be accompanied by a legal guardian.

We urge families to register early, as space is limited and is available on a first-come, first-served basis for those who are eligible. For your family to be considered, MDA must receive a completed application no later than one month prior to the start of the session.

A COMPLETED ULTRACAMP APPLICATION CONSISTS OF:

  • A recent photo of each family member
  • All application forms completed and signed electronically
  • A copy of each individual’s insurance card (if different)
  • A copy of each child’s immunization record and COVID-19 vaccination card (if separate)
  • Criminal Background Check - All adults ages 18 and older
