UltraCamp functions best on a computer and is not always compatible on tablets or mobile devices. If you do not have access to a computer, or need support in completing the application, please reach out to camp@mdausa.org. Language translation services are also available.
Thank you for your interest in MDA Summer Camp!
This week-long, residential camp program is available to children and young adults ages 8-17 living with neuromuscular disease, who are registered members with MDA. If not already members, applicants and their legal guardians will be automatically enrolled; there are no fees associated with membership. For more information, please visit

Campers may attend one session per year, either in-person or virtual. Guardians are encouraged to consider the child’s readiness to be away from home and their desire to go to summer camp, as well as their ability to participate in activities and enjoy the summer camp experience.
The UltraCamp camper application process is comprehensive and multi-step, and must be completed by the child’s legal guardian. If you are unable to complete the application in a single sitting, be sure to mark the forms as ‘Incomplete’ and click ‘Save’. The incomplete form will remain available in your UltraCamp account.
All application components must be submitted no later than six (6) weeks prior to the start of the camp session.
Submitting the UltraCamp application components does not guarantee acceptance.
Guardians will be notified whether their child is accepted after all applications have been reviewed and the necessary volunteer support has been confirmed.
If the number of camper applicants reaches the capacity for the session, additional applicants may join the waitlist. MDA will contact families if a spot becomes available. In these instances, MDA may contact guardians to confirm their interest and share their plan to submit any outstanding application components.

Medication Input
All medications brought to camp must be FDA approved, prescribed and medically necessary; documentation must include medication name, the route, schedule, dosage, the prescriber’s name, and the reason for the medication.

MDA Camper Health Form
Form is provided by the Camp Director and must be completed by a physician or licensed practitioner familiar with the camper applicant’s neuromuscular disease based on an examination that took place within 12 months prior to the start of the camp session.
