Terms and Conditions 2021

This agreement is made by and between the undersigned and custodial parent[s] or legal guardian[s] (collectively hereinafter, the 'Parent') of the camper(s)/student(s) identified above (hereinafter, the 'Camper' or 'Child(ren)') and Pacific Northern Academy Create Academy Programs (hereinafter, 'PNA', 'Camp' or 'Create Academy') for the enrollment of the Student in Create Academy.

By executing registration, Parent represents and warrants to PNA that he/she is, or they are, either the custodial parent of the Camper or the legal guardian of the Camper, and in either case, Parent has the right and authority to enter into this registration agreement. Parent certifies that all information completed in this form is correct and accurately reflects the child(ren) to whom it pertains.

By payment of any sum to PNA, you are acknowledging your agreement to the Terms and Conditions of PNA as described herein.

These Terms and Conditions set forth obligations applicable to both parents and children attending after school enrichment classes or school year or summer camp programs at Create Academy organized by Pacific Northern Academy:

GENERAL COMMITMENTS
Children in grades K-6 can participate in weekly Create Academy summer camps, June 1- July 30, 2021. Kindergarteners must be five (5) years of age as of September 1, 2021. Grades are calculated by the student's grade during the 2021-2022 school year.

Children in grades PreK (ages 4 or 5) through 8th grade are eligible to participate in after school enrichment classes.

After school enrichment classes meet one or two times per week depending on the class.

After school enrichment classes, School Break camp fees are required in full at the time of registration unless a payment plan is provided.

An annual $56 non-refundable registration fee is charged per student. The fee covers all activities between June 2021- May 2022.

Cancellation of school break camps must occur one month prior to the start of Thanksgiving break camp, winter break camp or spring break camps in order to receive a full refund minus a $50/ camp non refundable fee for each camps.

Cancellation for after school enrichment classes must occur one month prior to the start of the class in order to receive a full refund minus the deposit of $25 for classes that meet one time per week or $50 for classes that meet two times a week.

Cancellation must occur two weeks prior to the start of a weekly camp for summer camp in order to receive a refund. The $50/camp non refundable deposit is waived for 2021.

No refunds for a summer camp within two weeks of the start of the registered week of camp.

Summer camp may be paid on in full at the time of registration, on a monthly payment plan, or in two payments. Payment in full must occur two (2) weeks prior to the start of the camp. Two (2) weeks before the start of a registered camp the Parent will be fully liable for the payment of camp registration fees for that registered week.

Camps run Monday through Friday unless otherwise noted, and are filled on a first-come, first-served basis. If a payment plan is chosen at registration for summer camp, full payment must be received two(2) weeks prior to the start of the camp to secure your child's spot at camp.

Camp registration shall be considered accepted by PNA once this agreement is signed by the Parent and payment in full of the camp registration fee has occurred.

Parent will receive a confirmation email when the registration process has been completed. A reminder email with additional information will be sent in the week prior to the registered camp session.

Camp Tuition for each program is $310 unless noted. An optional $15 supply kit is available for purchase. The camp fee covers all programming/care between 7:00 a.m. and 6:00 p.m. After school enrichment class fees range from $75-$350 based on classroom hours. Additional technology/supply/lab fees may be charged for specific camps/classes.

Parent understands that additional costs beyond those covered by the camp registration fees may be incurred including, but not limited to, NSF fees, a late payment fee, late pickup charges. A late payment charge of $50.00 will be assessed if payments are not received five (5) days prior to the start of a weekly camp. Payment in full is required for all registrations occurring within two (2) weeks of the start of camp or at the time of registration for after school enrichment classes or break camps. Any account in arrears past 120 days may be sent to collections at the discretion of the School. The undersigned agrees to pay all costs of collection of amounts due under this agreement, including, without limitation, reasonable attorney fees, arbitration costs, and other costs.

Pacific Northern Academy reserves the right to withhold registration of campers whose accounts are not current.

By accepting this Terms and Conditions, Parent confirms that his/her child has permission to participate in all camp activities.

All Camper's personal belongings are his/her own responsibility; expensive and treasured items should not be brought to the camp. PNA is not responsible for lost, stolen, damaged or missing items.

No personal electronic equipment, including but not limited to: mobile phones, computers, TVs and video games are allowed to be used while at camp/ after school enrichment classes unless explicitly noted in the camp or class description. All technology needed at camp will be provided by PNA unless otherwise noted.

Parents agree to pay for property damage caused directly, or indirectly, as a result of negligent action of their child.

CONDUCT
It is Parent's responsibility to read and abide by the policies and procedures laid out in the Parent Handbook. Parent is required to read and explain to his/her child(ren) the guidelines encompassed by the Parent's Handbook that is available on PNA's website and in the confirmation email. The Camper's continued enrollment is dependent on the Camper's and Parent's compliance with the rules of conduct set forth in the Parent Handbook.

At the sole discretion of PNA, if the Camper or Parent fails to comply with the rules of conduct found in the Parent Handbook, PNA has the right to dismiss the Camper. If this occurs, no reduction or return of fee, or any part thereof will be made. Parents are responsible for the prompt removal of their child if their child is dismissed from camp or after school enrichment classes. This discretion may be exercised without the liability of PNA or its administration, faculty, or Board of Trustees.

HEALTH AND WELLNESS
By registering for camp, Parent accepts these Terms and Conditions and agrees to complete the health history form accurately and provide a complete record of immunizations that includes the date of the last tetanus shot, for his/her child or provide the appropriate signed exemption form. Failure to provide this information will prohibit the child from attending camp.

All prescription medications that are to be dispensed during the camp day/ or during after school enrichment classes must include the original packaging and prescription label with child's name and dosage listed. Medications must be given to the PNA staff on the first day of camp or each weekly class and will be dispensed by PNA staff only unless a written note from a doctor requires a camper to carry his/her own medication (Epipen, inhaler, etc.).

PHOTO RELEASE
By accepting these Terms and Conditions, parent confirms his/her acceptance that his/her child may be pictured/photographed and that such material can be used by Pacific Northern Academy and/or PNA's business partners for promotion or marketing of enrichment classes, camp and PNA, Create Academy or our early childhood program or other such programs.

Parent by his/her/their execution below, represents and warrants that it is the understanding of Parent that by completing this registration form, Parent gives PNA their permission to use Parents' and Students' first names and/or photographs in news releases, brochures, newsletters and other printed materials, audiovisual and web-based communications and other official PNA communications as may be developed and disseminated to communicate about PNA or Create Academy programs and Camper activities. Parent expressly releases PNA, its agents, employees, licensees, directors, trustees and assigns from and against any and all claims that Parent has or may have for invasion of privacy, defamation, or any other cause of action arising out of use, distribution, broadcast or other exhibition of Parent's of Student's name, likeness and/or photograph in official PNA communications.

The copyright of all photography appearing on PNA brochures or websites belongs to PNA. Reproduction may be made solely for home use without prior permission from the PNA administration.

Camper projects, such as stop motion animations, videos, photographs or artwork, etc. may be shared through a variety of public venues including, by not limited to, the PNA YouTube Channel, Facebook, Instagram or other social media sites.

If you would like to opt-out of all or part of the Photo Release permission, Parent must submit a signed letter stating your child's name, date and the extent to which you would like to remove permission. For example: Large group shots are approved but close up or small group photos are not.

CANCELLATION POLICY
If plans change and registration needs to be cancelled, all tuition will be refunded, minus the nonrefundable $56 annual per student registration fee, if PNA receives the cancellation request at least two weeks prior to the start of the registered summer camp.

No refunds within two (2) weeks of the start of the registered week of camp.

Please note that all registrations must be paid in full two (2) weeks prior to the start of the camp to secure your child's spot at camp.

No refunds will be given for after school enrichment classes if cancellation occurs within one (1) month of the start of the enrichment class.

No refunds will be given because of Camper absences, including illness.

If PNA does not have enough enrollment to hold a camp, PNA will refund all of the tuition for that camp/class.

Parent further understands that in the event that Camper is temporarily removed from PNA for disciplinary reasons, or camp or after school enrichment classes are canceled due to inclement weather or a natural disaster, or when necessary to protect the health and safety of campers and staff, Parent remains financially obligated for all camp fees and no refunds will be given.

By payment of any sum to PNA, you are acknowledging your agreement to the terms and Conditions of PNA as described herein.
